Florida's Year-1 award is approximately $209.9 million, its share of the federal $50 billion Rural Health Transformation Program. It is a five-year program; Florida's spending authority to expend the first-year federal award is carried in the state's Fiscal Year 2027 General Appropriations Act, and Year-1 dollars are being released through a slate of Requests for Applications administered by the Agency for Health Care Administration.

Florida's plan pushes care beyond traditional clinical walls — mobile health units, community paramedicine, remote patient monitoring, pharmacy-based retail clinics, satellite clinics and telehealth — while strengthening rural facilities' capacity for specialty and acute care, workforce pipelines, and value-based purchasing. A load-bearing technology thread is Florida HIE/ENS onboarding: the plan targets raising EHR and diagnostic interoperability among rural facilities from a 2024 baseline of about 73% to at least 83% by FY2030. Key project tags include Satellite Clinic Expansion, Mobile Maternal and Pharmacy Tech.

Florida structured its Year-1 funding as two bundled RFAs plus four standalone RFAs:

• Preventive Care & Care-at-Home Bundle (RFA 033-25/26) — five integrated initiatives (mobile units, community paramedicine, remote telemonitoring, pharmacy retail clinics, mandatory Florida HIE connectivity).
• Specialty & Acute Care Bundle (RFA 034-25/26) — HIE integration, diagnostic support, and a lead telehealth initiative (specialty, eICU, telestroke, or behavioral health).
• Value-Based Purchasing (RFA 035-25/26), Initiative 11.
• Rural Satellite Clinics (RFA 036-25/26), Initiative 1.
• Workforce Development (RFA 037-25/26), Initiative 8.
• Health & Lifestyle (RFA 038-25/26), Initiative 9.

Florida's first live procurement round was the six-RFA slate (033-25/26 through 038-25/26). The application due date was extended to Wednesday, June 17, 2026 at 2:00 p.m. EST. AHCA also published grant-writing resources for applicants and an expanded definition of “rural” eligibility (four qualifying pathways). The Rural Health Transformation Program was created by Section 71401 of the One Big Beautiful Bill Act (2025) and is administered federally by CMS. In Florida, spending authority flows through HB 5001-E, the Fiscal Year 2027 General Appropriations Act ($113.575 billion), which carries AHCA's authority to expend the state's ~$209 million first-year federal award; it was presented to Governor Ron DeSantis on June 23, 2026. Eligibility runs on two tracks. Direct-to-state contracts go out as competitive RFAs where firms bid as prime contractors. Subgrants and subawards flow to rural providers — hospitals, clinics and satellite sites, pharmacies, community paramedicine and EMS programs, and workforce/education partners — and it is here that technology and connectivity vendors most often engage as subcontractors or implementation partners, particularly around the mandatory Florida HIE/ENS onboarding component.
