The Rural Health Transformation Program (RHTP) is a $50 billion, five-year CMS program — created by Section 71401 of the One Big Beautiful Bill Act (2025) — that awarded every U.S. state a share to transform rural health care. Each state administers its own award through a designated agency, on its own timeline, with its own procurements.

Each profile collects the facts that are otherwise scattered across dozens of government sites: the state’s CMS award, its administering agency and contacts, its official program and procurement pages, and a dated log of activity — each entry deep-linked to the source.
