Indiana received approximately $206.9 million in Year 1 funding under the federal $50 billion Rural Health Transformation Program, part of the CMS awards announced December 29, 2025. As with every state, the award is disbursed by CMS annually over five years, with each year's payment tied to performance.

The Indiana Department of Health (IDOH) owns the program, branded GROW: Cultivating Hoosier Health. Competitive procurements are run for IDOH by the Indiana Department of Administration (IDOA), with IDOA's Mike Huth serving as buyer on several RHTP RFPs. Program contact: lbouza@health.in.gov.

GROW: Cultivating Hoosier Health brings the federal program to the state with a locally driven design — Indiana's premise is that rural communities know their own needs best, and it has stood up regional committees with dozens of members to shape local solutions. Early initiatives span a statewide Medical Operations Coordinator Center for transfer and EMS coordination, alternate Medicaid payment models (including a rural ACO), a rural clinical-training and preceptor pipeline, and a telehealth landscape assessment. Tracked priorities include cardiac access, pediatric telehealth, and a rural connectivity build-out.

Indiana's spending picture is emerging through its numbered RHTP initiatives rather than published percentage shares. Visible initiatives include Initiative 1 (Medical Operations Coordinator Center), Initiative 7 (Teleconsult/Telehealth Landscape Assessment), and Initiative 10 (Clinical Training and Readiness — rural physician and preceptor stipends plus a centralized preceptor registry), alongside an ACO feasibility study and a program-evaluator contract. Indiana issued a cluster of RHTP RFPs through the IDOA bids portal: RFP 26-87448 (MOCC Operations, a 5-year contract, due June 15); RFP 26-87556 (Clinical Training and Readiness / preceptor registry, Initiative 10, due June 15); RFP 26-87667 (ACO Feasibility Study, with a June 11 pre-proposal conference); RHTP07 (Teleconsult/Telehealth Landscape Assessment, due May 18); and an RHTP Admin Program Evaluator (due May 18). The Rural Health Transformation Program was created by Section 71401 of the One Big Beautiful Bill Act (2025) and is administered federally by CMS. Indiana administers its award through IDOH under its CMS-approved application and budget, with procurement run through IDOA; no separate enabling state statute is named in the record. Note a key structural rule Indiana added to its FAQ: primary subrecipients are prohibited from awarding funds to other subrecipients via sub-awards or sub-grants, though they may still engage contractors.

Eligibility runs on two tracks. Direct-to-state contracts go out as competitive IDOA RFPs, where firms bid as prime contractors — the route for most of Indiana's early administrative and infrastructure work (MOCC, evaluator, telehealth assessment, preceptor registry). Subgrants and subawards flow to rural providers — hospitals, Rural Health Clinics, FQHCs, EMS agencies and community organizations — where technology vendors and consultants most often engage as subcontractors or implementation partners. Note Indiana bars subrecipients from re-granting funds onward, so partners engage as contractors, not sub-subgrantees.
