Louisiana drew $208,374,448 in Year-1 funding under the CMS Rural Health Transformation Program — a top-five national award — and is positioned to pull down roughly $1.4 billion across the program's five years. It is Louisiana's share of the federal $50 billion program, with Year-1 dollars released through a fast-moving series of Notices of Funding Opportunity funded via Cooperative Endeavor Agreements.

The Louisiana Department of Health (LDH) administers the program through its new Office of Rural Health Transformation and Sustainability (ORHTS), which absorbed the former State Office of Rural Health. Program inquiries go to RuralHealthTransformation@la.gov.

Louisiana's plan is built around numbered initiatives spanning workforce, innovative and collaborative care, telehealth/access (MRAHA), and sustainable access to care. A centerpiece is a data-driven rural Medicaid Alternative Payment Model: working from a Milliman review of Medicaid claims, LDH found rural parishes run higher per-member cost but lower inpatient and readmission rates — a pattern it reads as a physical-access problem, not overutilization — and is funding a primary-care-led, provider-owned APM in response. Louisiana ran an unusually dense NOFO calendar and is now moving from solicitation into award. The Rural Clinician Credit Bank ($5.63M) closed June 25; Telehealth Infrastructure ($5M) and Capital Improvement ($39.9M) closed July 10; Collaborative Provider Models closed August 5; the Rural Medicaid APM (~$30M) closed August 7; and Food is Medicine and Regional Care Conveners closed August 14. Most Year-1 application windows have now closed (deadlines ran July 10, August 5, August 7 and August 14, 2026), and the state is in its award phase — watch for Notice of Intent to Contract announcements across late July into mid-August and RTCF Cohort 1 awards in mid-September. The Rural Health Transformation Program was created by Section 71401 of the One Big Beautiful Bill Act (2025) and is administered federally by CMS. In Louisiana, Gov. Jeff Landry signed an executive order creating the Office of Rural Health Transformation and Sustainability inside LDH and a RHTP Advisory Council that seats the chairs of the House and Senate Health and Welfare committees. The legislature added a standing oversight hook via Senate Resolution 198 (passed 36-0 on June 1, 2026), directing LDH to report to the legislature on its implementation of the award.

Eligibility runs on two tracks. Direct-to-state contracts go out as competitive NOFOs where firms bid as prime contractors. Subgrants and subawards flow to rural providers — hospitals, FQHCs/RHCs, ACOs and clinically integrated networks, EMS, and rural hospital-led partnerships. On the Medicaid APM, LDH is explicit that funding flows to Medicaid-enrolled provider entities and that vendors (it cites Equality Health and Azara as examples) can support implementation but are not the default recipients — the classic subcontractor / implementation-partner lane. The Rural Tech Catalyst Fund is a more directly vendor-facing vehicle.
