Maine received $190,008,051 in Year 1 funding under the federal $50 billion Rural Health Transformation Program. As with every state, the award is disbursed by CMS annually across five years, with each year's payment tied to performance; Maine adjusted its budget to meet federal requirements and the final award.

The Maine Department of Health and Human Services (DHHS) leads the program, in partnership with the Governor's Office of Policy Innovation and the Future. Maine is treating technology modernization as the foundation for everything else: its program leaders frame the work as groundwork for population health, augmented intelligence, telehealth, interoperability, and cybersecurity, starting with getting rural providers off aging electronic medical record systems. Maine's first provider funding opportunity is a roughly $30M EMR/EHR modernization program, and its structure is unusually explicit. The money splits three ways: about $9M to hospitals (minimum ~$306K rural, ~$204K non-rural per site), $20.4M to comprehensive care providers including primary care practices and CCBHCs (~$81K rural, ~$54K non-rural per site), and $600K to opioid treatment programs (~$42K/~$28K per site) — with rural sites receiving roughly 50% more than comparable non-rural sites. Maine's first provider opportunity is the ~$30M EMR/EHR funding program, and notably it is not an RFP: every eligible organization that submits a qualifying application is expected to receive an award, on a formula basis. Applications run through MCD Global Health and were expected to open in mid-July 2026. Eligible uses run wide — system replacements, upgrades, hardware, and the staff or consultant hours to implement any of it. The EMR/EHR application was expected to open in mid-July 2026 through MCD Global Health. Maine's RHTP Advisory Committee — providers, community organizations, Tribal partners, employers and educators — meets periodically to guide implementation (its most recent meeting was July 1, 2026, open to the public in listen-mode). The Rural Health Transformation Program was created by Section 71401 of the One Big Beautiful Bill Act (2025) and is administered federally by CMS. Maine administers its award through DHHS (with the Governor's Office of Policy Innovation and the Future) under its CMS-approved budget, adjusted to meet federal requirements and the final award; no separate enabling state statute is named in the record.

Eligibility runs on two tracks. Direct-to-state contracts go out competitively, where firms bid as prime contractors — and in Maine, MCD Global Health is itself the state's contracted RHTP administrator. Subgrants and formula awards flow to rural providers — hospitals, primary care practices, CCBHCs, opioid treatment programs, FQHCs and Tribal partners — where technology vendors and consultants most often engage as subcontractors or implementation partners. Maine's EMR program explicitly makes consultant and staff implementation hours an allowable use, a direct on-ramp for EHR, interoperability, and cybersecurity vendors.
