Mississippi's total RHTP award is roughly $206 million — precisely $205,990,180.16, and about 99.96% CMS-funded. This is Mississippi's Year-1 share of the federal $50 billion Rural Health Transformation Program, structured by CMS as an annual award across a five-year performance period (the program's obligation deadline for Year-1 funds is October 30, 2026).

The program is administered by the Mississippi State Department of Health (MSDH) through the Mississippi Rural Health Transformation Office, under the Mississippi Rural Health Transformation Program banner. Program contact: info@mississippirhtp.com.

Mississippi's plan concentrates on stabilizing and modernizing rural provider capacity — hospital stabilization, maternal health hubs, and care coordination — delivered through provider-facing grant programs across capital, technology, telehealth, workforce, and behavioral-health crisis care. A defining design choice: MSDH has deliberately built its applications to be completed without a grant writer, using pull-down selections and limited narrative responses so that providers, not consultants, fill out the forms.

Mississippi opened its first competitive phase in June 2026 with three provider grant programs making about $91.2 million available in Year 1:

• Rural Capital Project Care Gap Closure (RCGC) — $28.5M for facility renovations, upgrades, clinic expansion and equipment; 20–30 awards of up to $3M each (excludes new construction, telehealth, cybersecurity, EHRs, and broadband).
• Rural Provider Technology Grant (RTG) — $57M.
• Telehealth Hub Connectivity, Equipment, and Education (TCE) — telehealth adoption.

Mississippi's first funding phase (RCGC, RTG, and TCE) opened June 15 and closed July 15, 2026. A second wave followed: four Workforce Expansion Initiative (WEI) opportunities and the PES – EmPATH Units Grant Program opened July 15, 2026. On the direct-contract side, MSDH issued a Notice of Intent to Award on June 9, 2026 naming Premier Healthcare Solutions, Inc. as top-ranked offeror for the four-year Comprehensive State Health Plan contract (RFx 3180002944), from a deep field of seven responsive firms. Key posted dates: the WEI opportunities and the PES–EmPATH program opened July 15, 2026, with an application deadline of August 17, 2026 at 5:00 PM CST and EmPATH award announcements expected in September 2026. The Rural Health Transformation Program was created by Section 71401 of the One Big Beautiful Bill Act (2025) and is administered federally by CMS. Mississippi's legislature attempted to add competitive-bidding and reporting requirements through SB2477 and companion HB1067, but Governor Tate Reeves vetoed the bill on April 3, 2026, arguing (citing a CMS letter) that the added requirements would risk the state's ability to obligate funds by the October 30, 2026 deadline. As a result, the program continues to operate under MSDH's CMS-approved project and budget narratives without that statutory procurement overlay.

Eligibility runs on two tracks. Direct-to-state contracts go out as competitive RFPs (for example, the Comprehensive State Health Plan RFx), where firms bid as prime contractors. Subgrants and subawards flow to a broad pool of "Eligible Providers" — Mississippi-located hospitals (including critical access hospitals), physician practices and solo practitioners, rural health clinics, FQHCs, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, SUD and dental services, and licensed long-term care facilities located in or serving rural populations. Mississippi defines "rural" as counties under 50,000 residents or fewer than 500 people per square mile, or municipalities under 15,000; an organization need not be physically rural but must show meaningful benefit to rural Mississippians. Note the state's stated preference for providers over grant-writing consultants — UEI/MAGIC vendor numbers are required only before award execution, not to apply.
